Young Adult Project
The Continuum Care and Education Group’s Young Adult Projects are designed for those aged 18 – 25
years with complex needs.
Our Young Adult Projects have been specifically created for those in need of ongoing support
services and clinical input. Our specialist service aims to bridge the gap into adulthood by supporting
young people through the transition from teenager to adult. At Continuum we are committed to the
philosophy of normalisation. This underpins all of our work regardless of individual circumstances.
Registered by the Care Quality Commission, each of our Young Adult homes offers places for a
maximum of two residents and are supported by a highly qualified support team 24 hours a day.
Our residents’ days are busy and purposeful with access to further education and vocational studies
as well as any specialist clinical services they may require. The Continuum Group work in partnership
with Clinical Psychology Associates (CPA), an external company who provide a wide range of clinical
services tailored to meet individuals’ needs.
We have close links with external agencies such as Connexions and primary care trusts as well as local
police teams and safeguarding units to ensure the welfare of our residents.

Case study – Mike’s story
Mike moved into our Young Adults home just before his 18th birthday. His needs are complicated by clinical
depression, autistic spectrum disorder and Asperger’s syndrome. Communication is an issue for Mike. People find
it difficult to understand him and this can trigger frequent and extreme mood swings. Mike has made good progress in
his placement and has now found himself a job which means he has to be up and out of the house early.
His support workers also get up with him at 6.00am to encourage his enthusiasm. Mike has a very active daily
planner alongside an independence plan. The plans are in picture form so he understands what his day will entail.
This has given Mike a new lease of life and he is currently seeking to enrol on a mechanics course at college. Mike
has made lots of new friends in the local area and has started to attend groups and clubs for others in a similar
situation, who need that extra little bit of care and support as they move into adulthood.
