The Continuum Group has long experience of providing frontline support to young people and their families in
crisis. In recent years we have also developed an excellent track record for successful care leaver outcomes.
The development of our Family Support Services takes our commitment to each end of the continuum of care a
significant step further.
Our aim is to provide packages of support tailored to the needs of the individual child, young person or
family. Our bespoke services are developed in consultation with all the professionals and individuals
involved. These services include Young Adult Projects, Care Leaver Projects, Short Breaks and
Outreach Supervised Contact.

Care Leavers Accommodation Our care leaver packages meet the needs of young people aged 16 and over who are vulnerable and need support but are ready to develop independence skills.
Young Adult Projects Our Young Adult Projects have been specifically created for those in need of ongoing support services and clinical input. Ourspecialist service aims to bridge the gap into adulthood by supporting young people through the transition from teenager to adult.
Supervised and Supported Contact Facilitating more positive interaction between the young person, their family and the broader community.
Independence and Care Leavers Providing a supportive framework in the transition to self–sustained independence.
